RN Joint Replacement Program and Stroke Program Coordinator, Full-time, 7:30am - 4pm

The Total Joint Program Coordinator is primarily an administrative position. The coordinator serves as a resource person for diverse care needs of patients, assisting patients seamlessly navigate throughout the Joint Replacement Program with primary focus on patient safety and satisfaction. This position assists the director in all assigned duties including but not limited to program development, building patient and provider relations, data collection and marketing.

The Stroke Program Coordinator is responsible for the coordination of clinical, educational, planning, development, implementation, and evaluation of the stroke program. The Stroke Coordinator serves as a clinical resource person for all areas caring for stroke patients.. Assist with development and application of evidenced base nursing practice for high risk disease patient populations. Responsible for assisting coordination of Joint Commission Disease Specific Certification accreditation, rounding on stroke related diagnosis to ensure compliance with clinical outcome indicators.

Required Skills

• Analytical skills, knowledge of Joint Commission Disease Specific Certification and Center for Medicare and Medicaid requirements.
• Effective written/oral communications.
• Problem-solving skills and organization skills.
• Knowledge of continuous quality improvement tools, Plan Do Check Act performance improvement methodology and techniques.
• Personal computer skills and knowledge of applicable software programs.
• Ability to perform job related activities utilizing correct body mechanics
• Advanced Windows and Microsoft Office applications skills preferred

Required Experience

• Current Licensed clinician in California, RN
• BSN
• BLS
• NIHSS
• Minimum five years’ experience in acute care setting.

Preferred:

• Program development experience
• Administrative experience
• Masters of Science in Nursing
