Required Skills

• Provide care as an extension of the Family Medicine MD through clinic appointments, including:
• Physicals, PHASEs, Wellness Visits (WWE), routine care, sick visits, urgent care, and post-hospital discharge follow-ups
• Collaborate closely with the Family Medicine MD and Internal Medicine team to ensure high-quality patient care
• Coordinate care and referrals with specialists across the PIH Health network using a unified EMR system
• Educate patients on disease prevention, health management, and wellness strategies
• Manage a full patient caseload independently while maintaining strong communication with the care team
• Apply expertise in primary care, family medicine, and women’s health for patients of all ages
• Demonstrate effective teamwork in a multidisciplinary clinical environment

Required Experience

• Current California Nurse Practitioner (NP) or Physician Assistant (PA) license
• Graduate of an accredited Family Medicine NP/PA Master’s program
• Minimum of 3 years previous experience in Family Medicine
• Board certification as a Nurse Practitioner or Physician Assistant
• BLS certification from the American Heart Association
• Evidence of continuing education and professional development to maintain licensure
• Preferred: Proficiency in Spanish or Korean
