Required Skills
- Provide care as an extension of the Family Medicine MD through clinic appointments, including:
- Physicals, PHASEs, Wellness Visits (WWE), routine care, sick visits, urgent care, and post-hospital discharge follow-ups
- Collaborate closely with the Family Medicine MD and Internal Medicine team to ensure high-quality patient care
- Coordinate care and referrals with specialists across the PIH Health network using a unified EMR system
- Educate patients on disease prevention, health management, and wellness strategies
- Manage a full patient caseload independently while maintaining strong communication with the care team
- Apply expertise in primary care, family medicine, and women’s health for patients of all ages
- Demonstrate effective teamwork in a multidisciplinary clinical environment
Required Experience
- Current California Nurse Practitioner (NP) or Physician Assistant (PA) license
- Graduate of an accredited Family Medicine NP/PA Master’s program
- Minimum of 3 years previous experience in Family Medicine
- Board certification as a Nurse Practitioner or Physician Assistant
- BLS certification from the American Heart Association
- Evidence of continuing education and professional development to maintain licensure
- Preferred: Proficiency in Spanish or Korean
