This position involves outreach to members, assisting with care coordination, preparing case documentation, and supporting interdisciplinary team functions. The Case Management Coordinator will work closely with case managers, social workers, providers, and interdisciplinary teams to facilitate seamless care delivery. The Case Management Coordinator assists with the management of members in High Risk Case Management, Complex Case Management (CCM), Special Needs Plan Case Management (SNP) and Coordination of Care including Transitions of Care (TOC).

Required Experience
- High school diploma or GED
- Minimum of two years of experience in health care field
Preferred:
- Experience in case management, managed care, or healthcare coordination
