PIH Health is a leading nonprofit healthcare network serving the Los Angeles County, Orange County, and San Gabriel Valley region. We are currently seeking a Licensed Clinical Social Worker (LCSW) for our Ambulatory – Crisis team at PIH Health Whittier Hospital. This full-time, regular/at-will position offers a competitive hourly wage ranging from $44.00 to $67.00. As an LCSW in our crisis intervention team, you will provide critical consultation services to staff and other healthcare providers, serving as a vital link between our health network and the community. Your role will involve integrating services, coordinating patient care, and enhancing public relations through active community engagement. You will be responsible for assessing urgent or high-risk clinical situations, making appropriate interventions, and coordinating with Psychiatric Evaluation Teams (PET) when necessary. You will also assess mental health treatment needs and make suitable referrals, including coordinating patient referrals to psychiatrists or other mental health providers. Your expertise in conducting social work consultations and resource development will be essential in performing case management, coordination, and monitoring of social work service plans. Additional responsibilities include clinical supervision for unlicensed social workers, precepting MSW students, and providing education and training to the healthcare team on social work services, behavioral health, palliative care, hospice, end-of-life care, and regulatory requirements. You will assist with the administration, development, implementation, and evaluation of social work programs and policies. Your role will also involve participating in performance improvement initiatives, implementing Quality Improvement processes, and assisting with the development and implementation of the department's annual work-plan. You will perform clinical record audits, maintain educational records, provide evidence of competency, and participate in departmental functions such as staff meetings, in-services, and committees. Required Skills

  • Demonstrates skill in patient assessment, intervention, planning, and evaluation.
  • Demonstrates initiative, good judgment, and the ability to make decisions independently.
  • Demonstrates skill in working with patients having planned and unplanned life transitions such as but not limited to diagnosis of chronic and acute illness, death and dying, and complex family dynamics.
  • Strong communication skills, verbal, non-verbal, written, and oral.
  • Knowledge with regulatory agency requirements, legal policies, and protocol.
  • Strong critical thinking and problem-solving skills to identify and resolve problems in a timely manner.
  • Knowledge of managed care payment requirements and Medi-Cal and Medicare requirements.
  • Demonstrates skill in working with chronic care and complex patients including assessing and managing psychosocial aspects of pain, if assigned to Palliative Care.

Required Experience

  • Current California license as Licensed Clinical Social Worker issued by the California Board of Behavioral Sciences.
  • Master’s Degree from an Accredited School of Social Work.
  • Two or more years of work experience related to Medical Social Work (Hospital, Home Health, Hospice, or Palliative Care).
  • Must possess valid, unrestricted California Driver’s License and valid auto liability insurance.
  • Evidence of continuing education.
  • Experience with regulatory requirements related to reporting of child/elder abuse, domestic violence, and other legal requirements.
  • Cognizant of or ability to learn policies in assigned unit. Has knowledge of the Medicare CoPs for assigned unit (Hospital, Home Health, Hospice, or Palliative Care).
  • Obtain Accredited Case Management (ACM) certification within 3 years of employment, if assigned to Primary Care/Ambulatory or Home Health.
